2/23/2024 EnchantaVaughn Approved Behavior: Client was excited to talk to his counselor and happy to be in therapy. Client exhibited welcoming gestures and expressed cheerful words of affirmations. Client was in a good mood, as evidenced by his content and relaxed facial expressions. Client was cooperative and respectful toward mental health professional and parent. Client was supported for displaying positive social skills and getting along with his siblings. Client demonstrated productive and optimistic choices by taking care of his responsibilities and deciding to finish his chores during his free time. Mother reports at times she has to prompt him several times in an effort for him to comply with her request Behavior: Client was seen working quietly and staying on task. Client did not show any signs of stress or anxiety as he continue to display full understanding of his mental health condition at which mother reports at times when Intervention: QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP listened to client explain why he reacts in the manners that he does and why he feels as if his behavior is appropriate. QMHP modeled proper positive behavioral expectations for the client to administer. QMHP inquired of client’s current mood. QMHP utilized active listening skills as client spoke of his current mood. QMHP inquired about school activities, asking what he does after school to obtain any of his goals listed on his ISP. QMHP assisted client in exploring other coping strategies to utilize in the home. Intervention: QMHP isolated client from distractions to decrease disruptive behaviors and to reduce episodes. QMHP assisted client in developing more social skills. QMHP inquired about possible challenges client can experience with applying this skill. QMHP provided positive feedback. QMHP reflected on client goals and objectives. QMHP assisted client in normalizing the changes that are occurring at home and in the community in order to prepare him for additional learning and education. QMHP provided support and encouragement to ensure client was on track with achieving his goals and objectives without depending on his mom’s support. Response: Client expressed fairness and being able to compromise and improve with how he deals with problems in the home. Client responds well to praise and support. Client is acceptive of redirection. Client has improved with staying calm and utilizing relaxation skills in the home. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine in hopes that he will be a better individual. Response: Client understands that sessions are a privilege and he have to show good work ethic and follow along with the interventions. Client displays coping skills when faced with conflict due to deep meditation exercises. Client anger is moderate to intense, 2-3x per week. Client is working on walking away and telling an adult before responding to anger provoking situations. Client understands that problem solving starts with thinking before acting. Client displayed an understanding of ADHD and its symptoms and causes. Client was observed and reported eliminating personal conflicts at home.
